It is the responsibility of each respondent to examine the entire RFI, seek clarification in writing (inquiries), and examine all requirements for accuracy before submitting any questionnaire. Lack of care in preparing a questionnaire shall not be grounds for modifying or withdrawing it after the due date and time, nor shall it give rise to any Contract claim. Completed questionnaires should be submitted electronically. You can send them as an email attachment to Janine Locke at the email address above, or you can print, scan, and FAX them to the FAX number above.

Instructions to Open Zipped RFI Folder
The following zipped folder contains an Excel spreadsheet with an accompanying instructional document in PDF format related to each of three Requests for Information:

New Tax Assessment System for the Accounting Division
New Financial ERP System for the Accounting Division
New Claims Management System for the Special Fund Division

These Requests for Information are to assist the Industrial Commission of Arizona in determining how best to fulfill the requirements for the purchase and implementation of application systems to support the three areas in the list above. Please be advised that none of these is a formal solicitation but one may follow depending on the information collected from the applicable RFI. Should the ICA determine a formal solication is needed based on the responses to one of these RFIs, the applicable vendor questionnaire attached will determine a list of qualifying vendors to receive the formal solicitation. All of the documents for this RFI are in a single zipped folder. To access this zipped folder, right-click on its link above on this webpage, then, depending on your version of Windows, choose "Save As" or "Save Target As . . ." to save it to a location on your computer. Next, to open the folder at that location, double-click on its icon or filename on your computer and it will open in Windows Explorer to expose the files inside of it. The individual files that constitute the RFI can then be accessed using standard Windows techniques. the PDF files open in Adobe Reader.
